Make an Appointment

It is neccessary that we work by appointments. Unfortunately, emergencies do occur which occasionally cause delays in our schedule. However, we will try our best to honor your appointment time.

If you cannot keep a scheduled appointment, please give us at least a 24-hour notice so we can let someone else use your appointment time.

Please feel free to discuss fees with us, and tell us about any unusual financial problems.

Please inform us immediately of any change in your health history or medications that you are taking.

Please try to explain your needs and problems as accurately as possible. Because of the great number of daily phone calls, there is sometimes a possibilty of a breakdown in communication between patients and the office staff. If necessary, we will return your call as soon as possible.

Please give us complete and accurate information regarding your insurance coverage.

Please allow 1-2 working days for a reply to your appointment request by email. Please include the following information in your e-mail:

  • Full Name
  • Insurance Subscriber’s Full Name and date of birth
  • Insurance Policy Number
  • Insurance I.D. Number
  • Phone number and Physical Address
  • Reason for appointment
